Google is rolling out its Health Guardian suite through the Google Health app, bringing three new wellness-trend tools to the Pixel Watch 3, Pixel Watch 4 and Pixel Watch 5. Users can begin setting up blood pressure trends, insulin resistance trends and sleep breathing quality metrics, with the first monthly reports expected in early October 2026.
The features are included at no additional cost on those Pixel Watch models. Google says Fitbit Air users will gain access to the blood pressure and insulin resistance trends later in 2026 through a Google Health Premium subscription.

Monthly trends rather than clinical measurements
Health Guardian works in the background, using signals collected by a compatible wearable to identify longer-term physiological patterns. The most important distinction is that the tools do not turn a Pixel Watch into a replacement for clinical equipment.
Blood Pressure Trends uses optical and motion-sensor data to analyse pulse patterns over time. Google says the feature is calibration-free and provides a contextual report on the first day of each month instead of real-time blood-pressure values. A report requires at least five consecutive days of data within a week during the preceding month.
The company says its model was validated against 24-hour ambulatory blood-pressure monitoring, but the feature is intended only for general wellness. It does not diagnose or screen for hypertension, cannot guide medication changes and is not a substitute for a cuff-based monitor or professional advice.
Insulin resistance estimates without a blood sample
Insulin Resistance Trends combines signals including heart activity, sleep and movement to estimate changes in metabolic health. Google says it validated the metric against conventional laboratory results based on fasting glucose and insulin measurements.
The report is generated monthly when there are at least seven days of eligible data. It is not a glucose monitor, a diabetes screening tool or a basis for changing insulin, medication or treatment. Google positions it as a prompt to review lifestyle patterns or consult a healthcare professional when a sustained shift appears.
Sleep breathing quality provides daily and monthly context
Sleep Breathing Quality examines minute-by-minute blood-oxygen variation and motion during the user’s main sleep period. The app presents a daily percentage showing how much sleep was spent in what Google classifies as optimal breathing, then combines the data into a monthly assessment.
Google says the underlying approach was validated against clinical polysomnography. Even so, the metric is not intended to screen for or diagnose sleep apnoea or another medical condition. It is available only to users aged 18 and above.
Availability depends on device and region
Google says Health Guardian availability varies by country, device and subscription. The company’s announcement does not provide a complete country-by-country rollout list, so users should check the Google Health app on their account before relying on a specific feature being available.
The blood pressure and insulin resistance trend tools are not intended for people who are pregnant. None of the three features should be used to adjust medication or replace consultation with a qualified healthcare professional.
The broader Health Guardian suite also includes Breathing Emergency Detection for the Pixel Watch 4 and 5 in selected European markets. Google says it can prompt an emergency-services call after severe drops in blood oxygen, but notes that the feature has not been cleared or evaluated by the US Food and Drug Administration.
TTR has not tested Health Guardian or independently evaluated the accuracy of its trend reports. Sensor performance and the usefulness of the resulting insights will depend on wear time, data quality, device support and the user’s circumstances.
